• Upload your video to YouTube.com – (include your name and Karobics in the title of your video)
• Complete the entry process by submitting your entry form by email to contest@karobics.com
• Enter as often as you like. You will need to submit an entry form each time. Submission form can be found here
All submissions must be received by 5:00pm CST on December 31, 2009
If you do not have access to a computer for uploading, you can save your submission to CD or DVD and mail your submission with the submission form provided below

NO PURCHASE NECESSARY TO ENTER OR CLAIM PRIZE.
OFFERED ONLY IN THE 50 UNITED STATES AND DISTRICT OF COLUMBIA (“U.S.”)
AND OPEN ONLY TO LEGAL U.S. RESIDENTS WHO ARE 18 YEARS OF AGE OR OLDER.
All entries must be received on or before December 31, 2009. The selection will be held the week of January 1, 1010. Decision of the judges and Karobics Systems, LLC is final. The Prize winners will be notified by phone and/or mail and must meet all eligibility requirements.
By entering, each contestant agrees to be bound by the rules. Karobics Systems, LLC, its promotional agents and sweepstakes co-sponsors are not responsible for illegible, lost, delayed or misdirected mail, errors in faxes or shipments, busy phone signals, or incomplete entries.

Prize: (Three) Prize winner(s) will each receive Free Karobics Aerobic Self Defense – Core Effects 3 DVD Set plus Free Shipping and Handling. Approximate total value of the grand prize is $50.00 each. There are no other prizes.
The Grand Prize is non-transferable. There may be no cash or other substitutions of prizes by winner. Individual, parent or legal guardian of winner is solely responsible for all applicable federal, state and local taxes with respect to entire prize.
Winner or Parent or legal guardian of Grand Prize winner must sign an Affidavit of Eligibility, Release and Indemnification Form ("the Form") that : a) releases, indemnifies and holds harmless Karobics Systems, LLC, its parent, affiliated, subsidiary and successor companies, and its respective officers, directors, employees, agents, and representatives, advertising agencies and promotional sponsors (collectively the "Release") from and against any and all harm, expense, liability or injury that may be sustained relating to or arising from participation in the sweepstakes or acceptance or use of the prize; and b) authorizes the Releasee to use the winners' names, likenesses, photographs, voices and biographical data for any purpose, in any media, unless prohibited by law. All Submissions become the sole property of Karobics Systems, LLC for use in promotions and advertising.
Prize will not be released until Karobics Systems, LLC receives all required documents. Failure of Prize winners to return the Forms within the required timeframe will result in automatic forfeiture of any prizes and all such prizes shall be awarded to an alternate winner.
